How to Order Your Choose Life Plate

When ordering a Choose Life Plate, the DMV will apply any portion of the registration fee remaining on your current license to the new Choose Life license. You do not have to wait until your normal renewal date to obtain a Choose Life tag.

The cost of a Choose Life plate will be $50 for two years over and above the cost of your normal license registration. This money will be distributed to agencies which assist women with unplanned pregnancies and those considering adoption.

You will also pay a one time fee of $3.50 to the DMV.

There are several ways to order:

  1. DMV Website

    Visit this link on the DMV website, enter your license plate number and select the Choose Life License Plate under the “Special Interest” section. The amount you need to send will be calculated for you. Click “Ordering Details” at the bottom of the page. Then click “Order Form” at the bottom of the next page. Fill in all of the information, print the form, and then mail it to the address on the form along with a copy of your vehicle registration, a copy of your insurance card, and the amount indicated. The DMV will mail you the license plate.

  2. Visit the Department of Motor Vehicles Office near you.

    You will need to bring your vehicle registration, current proof of insurance, and a check or cash.  Please note that all DMV locations do not stock the Choose Life speciality plate at all times.  They might ask you to return once they are ordered.

  3. Call the LA Department of Motor Vehicles: (225) 925-6371

    When calling the DMV, you will give them your license plate number and they will tell you exactly how much to mail in for the Choose Life plate.

    You will mail in a copy of your vehicle registration, a copy of your current proof of insurance, and a check or money order to cover the cost of the new plate.
    Note: Often, the DMV number can be busy or and the automated system can be tough to navigate.  We encourage you to utilize the online method outlined in #2.
